Spinal Cord Injury Ontario (SCI Ontario) has 68 years of collective expertise in the

Job Summary
As part of a dynamic provincial organization, the Personal Support Attendant will be a member of a team that supports the delivery of quality client services that are consistent with our philosophy of independent living and client-directed care. This is a part-time position carrying a master schedule of up to 29.5 hours including weekends.

The Personal Support Attendant will:
 ·  Provide daily living assistance to adults with a physical disability living in their own
home
 ·  Assist with client directed duties such as personal grooming, dressing, bathing, meal
preparation, light homemaking
 ·  Travel throughout Toronto/GTA between clients
 ·  Support the philosophy of independent living

Qualifications:
     
  • PSW Certification
  • Vulnerable  sector screening & CPR/First Aid certified
  • Minimum  1 year relevant community outreach experience
  • Ability to work independently and take direction well
  • Must be able to communicate well in English, second language an
    asset
  • Access to a reliable vehicle, an asset
 
Eligible candidates must be able to work non-traditional hours including early mornings, evenings, weekends and holidays. Service hours vary from 1 hour to 3 hours

SEIU Service Wage Rate: $20.9818 - $21.5298

Unit Description
The Apotex is a wonderful state of the art building catering to the needs of the
elderly Jewish population. The Apotex is made up of 6 units. Each unit is
composed of three communities and each community is home to approximately 23 to
28 residents. Our primary population is residents who live with Alzheimer’s
disease or some form of dementia.

Responsibilities
 ·   Providing care to meet their needs as identified in the interdisciplinary care
plan.
 ·   Supporting their independence, dignity, social, emotional and cultural
well-being.
 ·   Initiating and maintaining a relationship with the client, their family and other care
  providers.
 ·   Promoting an environment that is conducive to dementia care.


Qualifications include:
 ·    P.S.W. certificate from an accredited community college or educational institute.
 ·    Current Basic Cardiac Life Support (‘BCLS’) certification is required.
 ·    Registration in good standing with the Ontario Personal Support Worker Registry, as may be required by the Ministry of Health & Long-Term Care.
 ·    Experience in a geriatric nursing facility.
 ·    Possess knowledge of the Long-Term Care Homes Act and Regulations.
 ·    Ability to deal with responsive behaviours.
 ·    Certificate in Montessori Method of Dementia Care is preferred.
 ·    Certificate in U-First is preferred.
 ·    Certificate from the Alzheimer’s Society for Dementia, Depression and Delirium is
preferred.
 ·     Ability to foster effective interpersonal relationships.
 ·     Demonstrated ability to provide client-centred care.
 ·     Basic computer skills an asset.
 ·     Demonstrated ability to meet professional clinical infection control
protocols.

As part of the Ontario Behaviour Supports System Project, Baycrest will be delivering a Long-Term Care Behavioural Support Transition Unit (‘BSU’) for individuals whose responsive behaviours have become unmanageable in their current setting, with a goal of stabilizing the behaviours allowing a return to a more appropriate destination, i.e. home, other LTC bed, home or community.
 
Baycrest’s innovative and exciting Behavioural Support Unit (‘BSU’) currently has an opportunity for a part-time temporary

PERSONAL SUPPORT WORKER/HEALTH CARE AIDE
who will provide care to clients requiring behavioural support and their families. 

Transitional Behavioural Support Unit – Apotex 3
Part-time temporary – 0.40 F.T.E. – 30 Hours Bi-weekly Approx. 12 Months
SEIU FT Wage Scale: $20.1671 - $20.6937

Working within the inter-professional team to provide collaborative and evidence-based care, responsibilities include:

Providing care and behavioural support to seniors with various types of responsive behaviours and their families. 
 
Providing patient care to the full scope of the PSW/HCA practice.

Working collaboratively with the interprofesional nursing team to ensure a supportive environment that enhances client care.


The successful candidates will have: Current PSW/HCA certificate from a recognized community college or education institute.



Grade XII Diploma or equivalent. 

Six (6) months experience in Long-Term Care with clients in day programs or residents in a Long-Term Care setting.

Previous Long Term Care and Community experience.

Current Basic Cardiac Life Support (‘BCLS’), level C is required.

Certificate in Dementia Care.

 Training in Gentle Persuasive Approach and U-First, an asset.

Demonstrated extensive knowledge and skill working with  individuals with various types of dementia, delirium, mental health, issues

Demonstrated familiarity with the Long-Term Care (‘LTC’) Home Act and Regulations, and specifically policies and procedures related to Abuse Free Environment, Restraints and Code of Conduct and Prevention of Violence in the workplace.

Familiarity with Resident Assessment Instrument (‘RAI’) documentation.

Demonstrated commitment to client focused care.

Commitment to the establishment of therapeutic relationships with clients and families.

Commitment to quality improvement, continuous improvement and change initiatives.

 Ability to implement best practices in behavior management.

Demonstrated trouble-shooting and problem solving approach relative to the situation and scope of practice. 
 
Ability to coach and mentor team members.

Ability to facilitate and take advantage of teachable moments during the provision of care.

Ability to work with minimal supervision.

Ability to work collaboratively, with respect and effective communications skills and interact with clients, family members, and members of the interprofesional care team, including community and primary team members.

 Basic computer skills and the ability to use electronic communication devices.

The PSW Instructor will provide students the skills they need to excel as a personal support worker in a private home or residential care facility. The PSW Instructor is responsible for leveraging their expertise and experience to deliver education services to students through:

- Delivering instruction that meets module and program objectives
as set out in the curriculum
- Preparing lesson plans and teaching aids that utilize a variety of teaching methods to promote student success
- Monitoring and discussing students’ progress with them individually and as a group
- Assigning and posting grades
- Participating in the development and evaluation of curriculum and instructional materials

To be successful in this role, you must possess the following education, experience and competencies:

- Must have a valid Registered Nurse (RN) license with the Registered Nurses’ Association of Ontario (RNAO)
- Minimum two years of related practical work experience as an RN
- Degree or Diploma from a recognized University or College relating to the area of instruction
- An equivalent combination of education and experience will be considered
- Strong communication and interpersonal skills, including presentation and group facilitation skills
- Ability to motivate, engage and inspire students
