We are currently looking for Personal Support Workers in the

In a home care setting you will assist clients with routine activities of daily
living and personal care. You will be responsible to provide home care to
clients in the areas of:
Quality of Life: relieving caregivers, teaching clients how to
live independently, advocating for client independence, encouraging clients to
try things to increase independence, playing games, talking,
reading.
Activities of Daily Living: waking clients up, bathing
and grooming, dressing, assisting with medication, toileting, shopping and meal
preparation, eating, housekeeping and laundry, monitoring client status,
ambulation and transporting client.

Qualifications: 
 ·   Completion of the PSW, or HSW Level II or III Certification Program.
 ·   Valid First Aid and CPR certification required.
 ·   Second language is an asset.

    Job Summary

    The PSW provides personal care and health maintenance to individuals and 
    families in the home setting within the framework of VON philosophy, policies, 
    procedures and standards, in community health care.
     
    The successful candidate will work in a Supports for Daily Living 
    [designated] apartment building.


    Responsibilities

    Collaboration with other health care team members is expected where and 
    when appropriate. At all times, the Personal Support Worker follows an 
    established plan of care and delegation of procedures that meets the needs of 
    the individual and family.
     
    The successful candidate must be able to work in a flexible care 
    program (working days/nights/weekends) and meet the On Call – Call Back by Pager  expectations required for this program.
     
    The successful candidate is expected to work every other weekend in 
    a 2 week rotation for the AM and PM shifts.
  • PSWs hired for VON Middlesex-Elgin may be required to work in multiple 
    programs.

    Qualifications
     
  • Personal Support Worker certification is required 
    Registration with the PSW Registry Ontario will be required:

    is a  non-profit, charitable organization which provides valuable, innovative, and caring support services to older and/or disabled adults who wish to maintain their quality of life while living in their own homes.

    Management, Homemaking, Meals on Wheels, Respite Care, Supportive Housing and

    St. Clair West Services for Seniors is seeking applicants for the contract position of Overnight  Home Support Worker
    Hourly rate for the position is $15.34 per hour. 
    Hours of work will be 9:45 pm – 7:15 am daily. 
    Shifts will be scheduled Monday – Sunday.  Full time hours can be obtained in 3-4 shifts.  This position is contract for the first six months with the intent to become permanent position within the organization.


    Job Summary:  The Overnight Home Support Worker provides
    overnight support to 5 supportive housing buildings.



     Primary duties include:
     ·  Responding to client calls from 5 supportive housing sites between  the hours of 9:45 pm and 7:15 am Monday - Sunday.  
     ·  Responding to client need for EMS service or other interventions overnight. 
     ·  Assist clients in their homes with personal care including bathing, dressing, toileting , incontinence care, ambulation and client repositioning on a scheduled and unscheduled basis 
     ·  Traveling between 5 supportive housing buildings as needed to respond to emergency calls and unscheduled client visits using own vehicle. 
    ·   Reporting clients’ behavioural changes, physical and safety hazards, task changes or other sources of concern to Manager of Home Support Services. 
    ·   Other tasks will be assigned to this position depending on client schedule and call volume. These tasks may be of an administrative or housekeeping nature.


    Qualifications:         
     
                                                                                                                                
     
    ·  Personal Support Worker Diploma or certificate with relevant experience
     ·  Eligible for enrolment in the PSW  Registry
     ·  Valid driver’s licence and own vehicle
     ·  Good written and verbal English language skills
     ·  Knowledge and experience working with seniors, frail elderly, chronically ill, cognitively impaired, and/or disabled individuals living in the community
     ·  Available to work overnight shifts Monday to Sunday

    preferences.  You will provide therapeutic and recreational support & will support the independence of the resident.  Duties: assist with providing
    activities, which are varied to meet the needs of the resident; assist the
    resident in integrating into programs which may include hobbies, social events,
    and therapeutic regimes; provide assistance for all activities of daily living;
    schedule & facilitate restorative care program/activities, medical
    appointments, meetings, social activities & care by other service workers,
    drawing on support from family members, therapists, community workers; drive
    household adapted vehicle; assist other caregivers, as required, with direction,
    information, or training; maintain good relationship and effective communication
    with the resident; assist in light meal preparation, snacks and menu planning
    & serve all meals and snacks to the resident; light housekeeping duties
    required; weekly grocery shopping.  
     
    This is a non-smoking, fragrance-free environment.

    Qualifications: One of: Personal Support Worker Certificate, Practical Registered
    Nurse, Rehabilitation Therapy Certificate, Degree in Therapeutic Recreation. 
    Previous work experience in a health care support environment is an asset.  Must have effective oral and written communication skills in English.

    speech impediment (aphasia) and needs assistance with all aspects of personal
    care, feeding, medication reminders/administration and transfers. 

    Duties include: Heavy lifting, Dressing/undressing, toileting, assist
    with bathing, light housekeeping, meal prep and planning. 

    Requirements: Physically strong, high school graduate, PSW training and
    certification. Car or transportation to client's home. Two former client
    references please. Will require assistance for up to 8 hours a day (this
    position will complement CCAC provided at-home care).

     Ideal candidate is a full-time paid caregiver with a PSW certification to
     accompany and support client and family through this difficult time.
     Familiarity with ALS is an asset. Experienced caring for a terminally ill patient and supporting the family with this process is an added bonus. Scheduling to be finalized but will complement the morning and evening/ overnight  community home care workers who help us part-time. Serious candidates need only to apply.
